If you are not sure what forms you need here is a list of some of the additional forms you may need. If you are not sure, please reach out to info@evergreenuwcd.org for assistance and we will help you through the process. 

As part of the application review process, EUWCD will verify that all wells on your property are properly permitted, registered as exempt, correctly abandoned, and accurately listed under your name.

Notice to Purchaser

This form is used to notify a buyer that the property they are purchasing is within EUWCD and explains the buyer’s responsibility to register or transfer ownership of any existing wells on the property within 45 days of the sale.

Change of Ownership Form

Fill out this form if you need to update the ownership information for an existing well.

Abandoned Well Form

Fill out this form to report a well that is (or soon will be) capped or plugged and is no longer in use.

Exempt Well Registration Form

Fill out this form and return it to EUWCD with payment and a state well report to register an exempt well.

Drilling Permit & Application

Fill out this form to apply for a permit to drill and produce from a new non-exempt well within the District. You will need to specify a driller and drilling specs.

Affidavit – Individual

Submit this affidavit if you are an individual property owner asserting your legal authority to access and produce groundwater. A notarized affidavit is required with the drilling permit & application. 

Affidavit – Entity

Submit this affidavit if the well is associated with a business, organization, or other legal entity with rights to access and produce groundwater. A notarized affidavit is required with the drilling permit & application. 

Monitor Well Application

Complete this form if you would like to designate an existing well for monitoring purposes and participate in EUWCD’s monitor well program. Learn more about our monitor well program here.
